CERTIFICATION:

I agree to accept responsibility in notifying the Bluegrass Community and Technical College and the Department of Veteran Affairs Office of ANY changes made in my student status. Included in this are changes in credit hours, major, class schedule and address. I understand that I cannot hold Bluegrass College or its designated officials professionally or personally responsible if certification of my enrollment is performed and I have provided false information to Bluegrass College or its designated officials.

I agree to comply with all rules of the Department of Veteran Affairs and Bluegrass College concerning the use of my VA benefits.

I understand that I should receive payment only while I am enrolled and attending classes at BCTC in courses that are required for my major area.

I realize that acceptance of additional money may constitute an overpayment which could result in my having to repay this amount to the Veterans Administration.

In addition, I realize that I cannot be paid for:

1. Taking a course that is not part of my curriculum plan (as listed with the VA).

2. Repeating a course to improve a passing grade.

3. Auditing a course.

4. Taking a course Pass/Fail.

Reminder: A schedule information card must be completed for every semester that the student is enrolled at BCTC. The student’s enrollment will not be “certified” until this form is completed and returned to VA Certifying Official in OB 203 at the Cooper Campus or at the Leestown Campus One-Stop in Building C-170.
