Please access the following forms which you will need to print and complete. Once forms are completed please schedule a time with the Human Resources Director, (859) 246-6704, to turn in completed paperwork.
 
Keep in mind that you should not begin work until your paperwork has been submitted to Jane Goatley, HR Director, and the appropriate documentation attached. As a reminder please bring your supporting documentation for the I-9 and a voided check.

o    The I-9 has been created by the Immigration and Naturalization Service (INS) to verify that an individual is indeed eligible to work in the U.S. Therefore, all employers are required by the government to obtain the information on this form before hiring an individual.

o    The W-4 form allows your employer to withhold the correct amount of Federal income tax from your salary.

o    The K-4 Employee's Withholding Allowance Certificate is a legal form that shows your withholding allowances for the state of Kentucky and is required by the state for tax purposes.

·         Direct Deposit Form (DOC / 58.0KB)

o    Direct deposit is mandatory. Direct deposit means that your paycheck is automatically deposited into your bank account. You need to fill out this form so that the College is authorized and has the correct information to make sure your paycheck can be deposited promptly on payday.

o    Payday is normally the 15th and 30th day of the month. Employees are paid on an arrears basis. (Example: Time worked the 1st through the 15th are paid on the 30th of that month; time worked the 16 through the end of the month are paid on the next month.)
