Employee Orientation 

Welcome to Employee Orientation  

Please access the following forms which you will need to print and complete. Once forms are completed please schedule a time with the Human Resources Director, (859) 246-6704, to turn in completed paperwork.   Keep in mind that you should not begin work until your paperwork has been submitted to Jane Goatley, HR Director, and the appropriate documentation attached. As a reminder please bring your supporting documentation for the I-9 and a voided check.

o    The I-9 has been created by the Immigration and Naturalization Service (INS) to verify that an individual is indeed eligible to work in the U.S. Therefore, all employers are required by the government to obtain the information on this form before hiring an individual.

o    The W-4 form allows your employer to withhold the correct amount of Federal income tax from your salary.

o    The K-4 Employee's Withholding Allowance Certificate is a legal form that shows your withholding allowances for the state of Kentucky and is required by the state for tax purposes.

o    Direct deposit is mandatory. Direct deposit means that your paycheck is automatically deposited into your bank account. You need to fill out this form so that the College is authorized and has the correct information to make sure your paycheck can be deposited promptly on payday.

o    Payday is normally the 15th and 30th day of the month. Employees are paid on an arrears basis. (Example: Time worked the 1st through the 15th are paid on the 30th of that month; time worked the 16 through the end of the month are paid on the next month.)

Benefits Information:

·         Tuition Waiver Form (DOC / 29KB)

o    Tuition Waivers for eligible employees are available for up to 18 credit hours per year (six hours) in the fall, spring, and summer) at any KY postsecondary public institution. Tuition waivers are also available to your spouse and dependent children (age 23 or less, as defined by federal financial aid guidelines) for six credit hours per semester (fall, spring, and summer) at any KCTCS institution.

o    Eligible employees who have been employed for a minimum of 1 (one) year or 1250 hours may be entitled to job protected family/medical leaves of absence for qualifying events as described under the act.

o    Eligible employees electing single health coverage or waiving health coverage will receive a monthly $50 benefit allowance. This allowance can be used in any combination to pay for benefit premiums not to exceed $50.

·         Employee Flexible Spending Account (FSA) and Health Reimbursement Arrangement (HRA) 

o    This reimbursement program allows employees to pay for qualified medical expenses and/or qualified childcare/dependent care expenses with tax-free earnings. Eligible employees who waive their health insurance can choose to have a portion of the employer contribution placed into their health care reimbursement account.
