Course-Related Forms

  • For Offering a Course Using a Distance Education Format – To be completed when the instructor and the student are not in the same location for at least 50% of the traditional class meeting time.
  • For a New Course (A100) – To be completed when a course has not been previously offered.
  • For a Course Revision (A200) – To be completed when there is a major revision of an existing course.
  • For a Minor Course Revision (A300) – To be completed when the present course will not be offered after the implementation of the proposed changes. Minor course changes are one or more of the following: Catalog number (course prefix/number – If number is being changed, please seek assistance from system office prior to change to determine if the number has previously been used. If new prefix is being requested, submit new prefix request form); Description (editorial change); Title (editorial change); and o Requisites (pre-requisite or co-requisite).
  • For Dropping a Course (A400) – To be completed to discontinue an existing course.
  • For General Education Status (A500) – To be completed to add or delete the General Education Status for an approved courses.
  • For Cultural Studies Status (A550) - To be completed to add or delete the Cultural Studies Status for an approved courses.
  • For Lower Division University Course (A600) – To request to offer a lower academic division course from a four-year state-supported college or university (See Rules of the Senate Section III, 2.3.).

Program-Related Forms

  • For a New Program – To be completed when a new degree, diploma, and/or certificate program is being proposed.
  • For a Major Change to an Existing Program – To be completed when one or more major changes are proposed in an existing degree, diploma, and/or certificate program.
  • For a Minor Change to an Existing Program – To be completed when one or more minor changes (e.g., curriculum title, program description, and/or program competency assessment) are proposed in an existing degree, diploma, and/or certificate program.
  • For a New Certificate – To be completed when a new certificate is being proposed.
